Service RequestNote: We will need to have a signed version of this completed before the event. 1st Source Plus, Inc. agrees to provide services, as desired and agreed upon by Client, in the form of master of ceremonies and/or entertainment with quality music (and lights/fog machine as defined in package selected). The DJ will set up a miniuim half-hour before event begins. In consideration for the above mentioned services, the Client agrees to pay the deposit in advance of the event. In addition, the Client agress to pay the balance of the contract sum to the DJ upon arrival on the event date before services are rendered. In the event that Client fails to pay for services in accordance with the terms of contract, the Clients forfeits the $100.00 deposit and agress to pay remaining balance and all attorny's fees necessary to collect the total contract fee. The Client agrees to be completely responsible for securing the facility where the event will be held as well as any permits, licenses, and/or authorization that are required to allow 1st Source Plus, Inc. to execute the terms of the agreement. Minimum deposit of $100.00 must be received with signed contract before event date is guaranteed. The $100.00 deposit is non- refundable except in the case where services cannot be performed due to Acts of God, in which case the contract terms then become null and void. If the client decides to purchase additional hours along with a package, the payment must be paid to the DJ before the additional extra hours are provided. There will be a $30.00 charge on all returned checks. Should any damages occur to the equpiment owned by the DJ or injuries be inflicted to the DJ as a result of delberate or negligated behavior or omissions of the Client, or his authorized representative(s), agent(s), or quest(s), then the Client shall be obligated to pay the full amount of damages.  |
